Yeah so you see it the same way darkrider2008 sees it. I lean more towards my way not just because of what you see visually but listen to the lyrics "It could've been should've been the worse thing you'd ever know" basically that means he wants it to be horrible he wants to talk about how his story is the worst his life is the worst. Basically the concept of the song is people feel too sorry for themselves, and really shouldn't.

No, the people at the bar don't believe Issac that there was a monster that attacked him, and turned his limb into a musical instrument, so the bartender showed the other people his leg.

My interpretation of it is basically the concept of the song is basically things could always be worse and people like to make their life or events in their life seem the worst when they aren't, so it's basically people telling their story so Isaac comes in telling this story of what happened to him and his hand, and then the bartender gets pissed because he thinks his life or story was worse which is why he looks angry and shows him the guitar leg.
